Payment in full must accompany this registration form. Cancellations must be made in writing. Written notice of cancellations received at NACT headquarters by April 30, 2017 are entitled to a full refund, less a $250.00 service charge. Registration cancellations received on or after April 30, 2017 are nonrefundable. If you are paying by credit card and fax your registration, please do not also submit your form online — this may result in double payment.
